About Choice Health Care

Putting People at the Heart of Everything We Do

Choice Health Care Limited is a specialist supported living and domiciliary care provider, based at Fairgate House, Tyseley, Birmingham, and serving communities across the West Midlands. We were founded on a simple belief: every adult deserves excellent care — delivered with dignity, warmth, and clinical expertise. Incorporated in October 2021 and registered with the CQC, we are a growing provider with a clear commitment to quality from day one.

We support adults with complex needs including forensic mental health conditions, learning disabilities, autism spectrum conditions, acquired brain injury, physical disabilities, and substance misuse recovery — from a few hours of outreach support to full 24/7 residential placements.

Our approach is trauma-informed, recovery-focused, and relentlessly person-centred. We don't fit people into packages — we design packages around people.

Choice Health Care Limited is registered with and regulated by the Care Quality Commission (CQC) — the independent regulator of all health and social care services in England. We operate in full compliance with the Health and Social Care Act 2008 and the CQC's Fundamental Standards.

We are a newly registered provider. CQC has confirmed, following their registration checks, that our service was assessed as likely to be safe, effective, caring, responsive, and well-led. Our first full CQC inspection will be published in due course — and we welcome this as part of our full commitment to transparency.

Our commissioning team responds within 4 working hours of referral receipt. Standard needs assessments are completed within 48–72 hours. For emergency or urgent placements we aim to confirm within 24 hours where capacity exists. We accept step-down, respite, remand, and emergency referrals.

We accept referrals from local authority care managers, NHS hospital discharge teams, community mental health teams (CMHT), probation services, MAPPA coordinators, assertive outreach teams, independent social workers, and self-funded individuals and families.
